The Health Planning Council of Southwest Florida, Inc., in collaboration with the Florida Diabetes Alliance, Big Bend Health Council, and WellFlorida Council, is pleased to announce that the following agencies have been selected to participate in the mentorship cohort program to develop or expand DSMES services for 2026/2027.

We will be issuing another application for the mentorship cohort program in the summer of 2027. If you would like to learn more, please email [email protected].
