According to the Public Health Accreditation Board (PHAB) a community health assessment is a comprehensive picture of a community’s current health status, factors contributing to higher health risks or poorer health outcomes, and community resources available to improve health. 

Elements of a CHA include:

  • Analysis of health data
  • Community survey
  • Focus group
  • Community leader interview

This year’s CHA was a collaborative effort by a steering committee composed of four key health organizations in Collier County: NCH, the Florida Department of Health in Collier County (DOH-Collier), the Healthcare Network (HN), and the Health Planning Council of Southwest Florida, Inc. (HPC).

The assessment utilized a variety of data sources, including demographic, socioeconomic, and health status information. It also incorporated valuable qualitative data from community input, such as surveys, interviews with community leaders, and focus groups.

Executive Summary

An Executive Summary for a Community Health Assessment (CHA) is a concise, high-level overview of the entire, often lengthy, report. Its main purpose is to provide a snapshot of the key findings, conclusions, and recommendations so that readers—especially those who may not have the time to read the full document—can quickly understand the most important information.

Community Input

The Community Input section is all about the community’s voice and is a crucial part of the Community Health Assessment (CHA). This document includes the results from residents and community leaders who share their perspectives, experiences, and concerns about health and well-being in their area through a community survey, interviews, questionnaires, and a focus group. 

Needs Assessment

A secondary data needs assessment is a crucial component of a Community Health Assessment (CHA) that focuses on compiling and analyzing existing health data from reliable sources. The section headers are:

  • Population
  • Drivers of Health
  • Settings and Systems
  • Health Behaviors
  • Health Conditions
  • Federally Qualified Health Clinic (FQHC) Data

This data provides a base of information to better understand the community, its people, and their health needs.
