Keeping Our Finger on the Pulse of Our Community

The Health Planning Department partners with local agencies to improve the health of SWFL, particularly those who are most vulnerable. Projects include strategic planning, community health assessments, project & contract management, data collection & analysis, grant writing, meeting facilitation, community surveys & focus groups, and more.

Area 8

Certificate
of Need

HPCSWF is under contract with the Agency for Healthcare Administration (AHCA) to provide program support for the Florida Certificate of Need (CON) Program. Such support includes conducting public hearings on proposed CON projects, monitoring approved projects and filing status reports to the AHCA.

Hospital and Nursing Home Utilization Data

Monthly utilization data from licensed hospitals and nursing home facilities in all seven counties is collected and analyzed by facility each month and is available for annual subscribers. This data is invaluable to hospitals, nursing homes, and consultants in determining market share and identifying trends.

Published County Health Profiles in each county in our region showing key health status indicators and trends in our area.
Created Needs Assessments, and Service Delivery Planning for persons with HIV/AIDS in our area.
Developed the HIV Prevention Plan for our region.
Formed the Collier County Health Care Access Coalition.
Organized the Disaster Planning for Persons with Disabilities Workshop.
Completed Community Health Assessments for Charlotte, Collier, DeSoto, Hendry, Glades and Lee Counties.
