Now Accepting Applications: DSMES Mentorship Cohort Program

We are pleased to announce that the Diabetes Self-Management Education and Support (DSMES) Mentorship Cohort program is now accepting applications for its upcoming cycle.

The core of this program—and its greatest value—is the opportunity to engage in a dedicated, collaborative mentorship network. By joining this cohort, successful applicants will receive resource support and get direct, tailored guidance to navigate the complexities of establishing, building, accrediting, sustaining, or expanding DSMES services for adults in Florida.

Why Join the Cohort?

  • Unmatched Mentorship & Guidance: The cornerstone of this experience is individualized agency mentorship. You will work closely with an experienced mentor as your agency builds and/or expands its DSMES services.
  • Technical Assistance: Receive targeted support in a group setting to help your organization build lasting capacity and streamline the path toward accreditation or recognition.
  • Financial Resource Support: Participants will be eligible to request $1,000 to $4,000 in funding to support their program’s capacity-building efforts.

The program period will run from approximately August 25, 2026, to May 7, 2027.

Our short-term goal is to put DSMES programs on a clear path toward national recognition or accreditation, and our long-term vision is to expand sustainable, high-quality diabetes support across Florida. This opportunity places a particular focus on:

  • Rural communities currently lack a recognized or accredited DSMES program.
  • Communities and populations experiencing disproportionately high rates of type 2 diabetes, complications, and diabetes-related mortality.
