The Health Planning Department partners with local agencies to improve the health of SWFL, particularly those who are most vulnerable. Projects include strategic planning, community health assessments, project & contract management, data collection & analysis, grant writing, meeting facilitation, community surveys & focus groups, and more.

Updated Access to Health Insurance Fact Sheet Now Available
“Focus On” is a series of fact sheets on health-related issues for Southwest Florida. A fact sheet is created for each of the seven counties in our area: Charlotte, Collier, Glades, Hendry, Lee, and Sarasota.
We hope they may stimulate conversation and perhaps encourage action in our community.
2024 County Health Profiles Now Available
The new 2024 Area 8 County Health Profiles are now available. Profiles are available for Charlotte, Collier, DeSoto, Glades, Hendry, Lee, and Sarasota Counties. The County Health Profiles aims to provide an overall snapshot of the county. Data includes:

- Health Indicators
- Healthcare Access
- Community Context
Area 8
Certificate
of Need
HPCSWF is under contract with the Agency for Healthcare Administration (AHCA) to provide program support for the Florida Certificate of Need (CON) Program. Such support includes conducting public hearings on proposed CON projects, monitoring approved projects and filing status reports to the AHCA.
