NOTICE TO ALL EMPLOYEES AND FORMER EMPLOYEES REGARDING 2025 ACA FORMS 1095-C AND 1095-B

The Health Planning Council of Southwest Florida, Inc. will be applying the Alternative Furnishing Method for 2025 1095 forms. 

Reporting entities are no longer required to automatically furnish Forms 1095-B and 1095-C to covered individuals.  Instead, reporting entities may post a notice on their website informing individuals that they can request a copy of the statement.

This shall serve as a clear and conspicuous posting of this Alternative Furnishing Method.   If you would like to request a copy of your 1095 form for 2025, please contact Human Resources directly at 239-433-6700 or email at HR@hpcswf.com.  You may also mail your request via USPS to us at:  Health Planning Council of Southwest Florida, Inc., Attn:  Human Resources, 12220 Towne Lake Drive, Suite, 1, Fort Myers, FL  33913.
