Diabetes Self-Management Education and Support (DSMES) Mentorship Cohort 25/26
The Health Planning Council of Southwest Florida, Inc., in collaboration with the Florida Diabetes Alliance, is pleased to announce that the following agencies have been chosen to participate in the mentorship cohort program for the development or expansion of DSMES services for 2025/2026.
- DeSoto Memorial Hospital
- Family Health Centers of Southwest Florida
- Healthcare Network
- Heart of Florida Health Center
- Meridian Healthcare
We will be issuing another application for the mentorship cohort program in the summer of 2026. If you would like to learn more, please send an email to Planning@hpcswf.com. In the meantime, we will be sharing educational materials related to DSMES throughout the year.
