Notice is hereby given that the Board of Directors of the Health Planning Council of Southwest Florida, Inc. will hold its meeting on Wednesday, April 8, 2026, from 10:00 am until 12:00 pm at the Health Planning Council of Southwest Florida, Inc. office located at 12220 Towne Lake Dr., Suite 1, Fort Myers, FL 33913.
The Health Planning Council of Southwest Florida, Inc. will be applying the Alternative Furnishing Method for 2025 1095 forms.
Reporting entities are no longer required to automatically furnish Forms 1095-B and 1095-C to covered individuals. Instead, reporting entities may post a notice on their website informing individuals that they can request a copy of the statement.
This shall serve as a clear and conspicuous posting of this Alternative Furnishing Method. If you would like to request a copy of your 1095 form for 2025, please contact Human Resources directly at 239-433-6700 or email at HR@hpcswf.com.
